What You Need to Know.

IMPORTANT! THESE TERMS AND CONDITIONS ARE A BINDING LEGAL AGREEMENT (this “Agreement”). PLEASE READ THESE TERMS AND CONDITIONS CAREFULLY BEFORE ACCEPTING THEM.

Young Starz Soccer/Futsal(Young Starz) event activities (hereinafter (“event” or "Activities") are available only to individuals who can form legally binding contracts. You can accept this Agreement for yourself if you are 18 or older (if the age of majority is older than 18 in your jurisdiction), otherwise a parent or legal guardian is required to accept this Agreement on any person attending the event with you.

By signing your full name, as the Parent/Legal Guardian of the Participant, you acknowledge reading and accepting the
Waiver of Liability and Release.

As the parent or legal guardian of the above named Participant, a minor, I understand the inherent risk of injury when participating in sports activities, and I hereby release, hold harmless, and forever discharge the Young Starz Soccer/Futsal(Young Starz), its coaches, volunteers, principals, employees, and agents (the "Releases"), and each of them, from any and all injuries, damages, claims, demands, causes of action, judgments, and liabilities, in law or in equity, however arising, known or unknown, now and in the future and to the fullest extent permitted by law, against the Releases and which may arise out of or as a result of the named Participant's participation in sports activities, including their transportation from and to the location of soccer/futsal activities, which transportation and participation I hereby authorize.

I grant to Young Starz Soccer/Futsal(Young Starz), its representatives and employees the right to take photographs & videos of me and my property in connection with the above-identified subject. I authorize Young Starz Soccer/Futsal(Young Starz), its assigns and transferees to copyright, use and publish the same in print and/or electronically.
I agree that Young Starz Soccer/Futsal(Young Starz) may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Young Starz Soccer/Futsal (Young Starz) is following all state guidelines for preventative measures to reduce the spread of COVID-19; however, Young Starz cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, training with Young Starz could increase your risk and your child(ren)’s risk of contracting COVID-19.By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by participating with Young Starz and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I understand that the risk of becoming exposed to or infected by COVID-19 while with Young Starz may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Young Starz employees, volunteers, and program participants and their families.I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance with Young Starz or participation in Young Starz programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Young Starz, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Young Starz, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Young Starz program.

REFUND AND CANCELLATION POLICY

Hosting high quality futsal league involves securing playing venues by way of prepaid rental and related fees well in advance of the actual event in which a player or team participates. As such, Young Starz hereby advises you in advance of its strict Refund and Cancellation Policy to which you will be bound if you proceed below and click “ACCEPT” and thereafter register either yourself (if you are 18 years or older), your child or your team to participate in one or more of Young Starz futsal leagues, futsal tournaments or futsal events.

All Team Fees received by Young Starz are 100% non-refundable. Once a Team is Accepted into the Event to which it applied NO REFUND of the Team Fee will be given if the team decides not to participate in the Event or is unable to participate for any reason.

Team Fees are not transferable

Refunds Due to Cancellation of Event by Young Starz
In the event Young Starz must cancel an Event due to circumstances outside of its control, including, but not limited to, inclement weather, a natural disaster, a pandemic, a government shutdown, Young Starz will refund 100% of the Team fees, less merchant processing an transaction.

IN CLICKING TO ACCEPT THIS AGREEMENT, YOU REPRESENT THAT (A) YOU HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT YOU
HAVE GIVEN UP SUBSTANTIAL RIGHTS BY ACCEPTING IT; (B) HAVE ENTERED INTO THE AGREEMENT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY
NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW; AND (C) AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
